Yes, you must have a written order that includes diagnosis (ICD10 code), tests ordered, and provider signature. You can bring a hard copy of the lab order to the Hoaglin Wellness Center or the medical provider can fax it directly to us at 614-366-9489.

There are several ways to travel to an off-campus appointment:

• Call Licking County Transit at 740-670-5185 and press #1 (or 1-800-350-7071) to schedule a ride.
• Use a ride-sharing platform such as Uber or Lyft.
• Call Campus Safety to schedule transport, if available: 740-587-0810
• Register for Enterprise CarShare Program.

Like any other clinic or doctor¡¯s office, at appointment check-in students, faculty, and staff will provide their insurance card, along with a form of payment for copay (no cash or checks accepted). Insurance will be billed directly. Counseling services are billed after the sixth visit and all psychiatrist visits are billed.

• Your nurse practitioner or physician will provide a treatment plan for you.
• You will be directed to register with MyChart.
• The bill for your visit will be submitted to the insurance you provided at check in.
• Once the bill is processed with insurance, any amount that you are responsible for (ex. co-insurance, deductible) will be billed to you directly from Ohio State Wexner Medical Center. The bill will come to the mailing address you provided at check in.

Same day appointments are dependent on counselor availability. Crisis Clinic appointments are available Monday through Friday between 2 - 4 p.m. for imminent mental health crises. We ask that students in crisis arrive by 3:20 p.m. to allow appropriate time to address their concerns. These appointments are brief stabilization visits reserved for students experiencing acute distress related to events such as suicidal thoughts/behaviors, acute trauma, and other safety concerns. In addition, all enrolled ±Ø²©ÓéÀÖ,±È²©ÓéÀÖÍøÖ· ######### students always have access to the ±Ø²©ÓéÀÖ,±È²©ÓéÀÖÍøÖ· ######### 24/7 on-call counseling service at 740-587-6647. Students can call this service anytime, from anywhere.

You can call ±Ø²©ÓéÀÖ,±È²©ÓéÀÖÍøÖ· #########¡¯s 24/7 On-call Counseling Service at (740) 587-6647. These calls are free and confidential within the Hoaglin Behavioral Health Team. You can use this number at any point during the year (including academic breaks) and access it while at home, on campus, or studying abroad.

Counseling services are confidential. This means that we will not confirm or deny your appointment attendance or what you share with a clinician without your expressed consent. If you want us to share information with parents, professors, or others, you can sign a Release of Information form that specifies what information we can share and with whom we can share it.

Limitations to confidentiality mainly involve safety-related concerns, and include:

• Imminent and foreseeable risk of suicide/ending your own life
• Imminent and foreseeable risk of harm to others
• Disclosures of abuse or neglect of a child, an elderly or disabled individual, or animal
• Disclosures of hazing as defined by Collin¡¯s Law and ±Ø²©ÓéÀÖ,±È²©ÓéÀÖÍøÖ· #########¡¯s Hazing Prevention Policy
• As directed by subpoenas/court orders

You may choose to meet with counseling on an as-needed basis or have ongoing counseling through the wellness center. Frequency of ongoing sessions typically does not exceed every other week.

We recognize that sometimes you may not feel connected with your initial counselor. We want to help you develop self-advocacy, and so we encourage you to share feedback with your therapist about approaches or techniques that are helpful and unhelpful for you. This feedback allows us to adjust our approach and also helps us to pair you with another available clinician as desired.

You can access information about your appointments in the MyChart app. If you miss a class due to an appointment, you can access and share your appointment information with the professor.

Based on guidance from the state licensure board, the Counseling Services staff cannot provide letters of support for ESAs. View the Emotional Support Animal Policy.

Although faculty or parents cannot schedule an appointment for a student, you may choose to have a student call to schedule an appointment during your meeting with them (office hours, advising meetings, etc.). If you need to make an immediate referral for a student in crisis, you can call ±Ø²©ÓéÀÖ,±È²©ÓéÀÖÍøÖ· #########¡¯s 24/7 on-call counseling service at (740) 587-6647, contact Campus Safety to see if a ride can be arranged, and encourage the student to access urgent walk-in hours at Hoaglin from 2¨C4 p.m. every weekday. We ask that you please provide us with a brief consultation or reason for the referral before our meeting with the student so that we can fully assess a student¡¯s level of need and risk of harm concerns.
